I consent to sharing my name and date of birth to confirm my identity. Providing this information allows the facilitation of communication and the provision of care through virtual and other communication tools.

I am aware of the potential risks associated with unauthorized disclosure or interception of personal health information. Should I have concerns, I will ask question about the steps I can take to help protect my information during our virtual interactions.

I understand that while care provided through video or audio communication is valuable, it cannot replace the necessity for a physical examination, or an in-person visit for certain disorders or urgent problems. I commit to seeking urgent care in the emergency department as necessary and if recommended.

We care about your privacy. When you register, you agree to get emails about your appointment. We won't share any medical details by email or with other people.

Aggressive behaviour

Here at the East Regional Virtual Care Clinic, we prioritize a safe and respectful environment for all clients and staff. Verbal abuse or aggressive behavior towards our dedicated healthcare professionals will not be tolerated and may result in the immediate termination of your appointment.

We appreciate your cooperation in creating a positive and constructive atmosphere for everyone involved in your care. What happens if your appointment starts and it’s deemed not appropriate for virtual care:

During the course of your virtual care appointment, it may become evident that the nature of your concerns requires an in-person assessment or immediate attention that virtual care cannot provide. In such instances, our dedicated healthcare professionals may recommend a referral to another medical service, such as your Primary Care Provider or your local Emergency Department.

We appreciate your understanding and flexibility in these situations. Our goal is to ensure that you receive the most appropriate level of care tailored to your individual needs. Your well-being is our top priority, and any recommendation for a different level of care is made with your best interests in mind.
